Signs of an Unhealthy Rabbit
First of all, rabbits are amazingly adept at hiding their illnesses. As a prey animal, they must be this way to avoid being picked out as an easy meal. It's up to you, the owner, to get to know your rabbit so personally that you can tell when one tiny thing is off and your rabbit is not feeling like itself.
Below are common signs that your rabbit is sick. However, all these depend on what your rabbit is sick with. If it is showing any of these signs, take to a vet immediately.

EYES - Watery/runny eyes; goopy eyes; itchy; red; swollen
NOSE - Runny nose; sneezing. Make sure you determine why your rabbit is sneezing; the rabbit to be allergic to something or could've gotten something in its nose. Not every sneeze is a bad sign. Clear discharge from the nose is usually a sign of allergies; colored discharge is something much more serious.
EARS - Black crustiness; bad odor
MOUTH - Warts; scabs; swollen, blue (or purple) lips (indicates breathing issues, emergency!)
FRONT PAWS - Matting; Wetness (because of wiping nose)
BACK PAWS - Sores; swelling
GENITALS - Scabs; Sores; Swelling
DIGESTIVE - Not eating; Bloated; Firm belly; Not drinking; Runny, watery poop (diarrhea)
